To become a Family Together Alliance Care Provider:
1. Complete Application and submit to FTA with necessary documentation (i.e.: state issued ID, social security card, et al)
2. Complete CPR/First Aid training and submit certification
3. FTA team will complete necessary background checks
4. Once background checks are completed, FTA team will reach out to care provider and complete onboarding training and answer any questions

What is a PASA?
A Program Approved Service Agency (PASA) is an agency that has been approved to provide direct community-based services to individuals with intellectual or developmental disabilities approved for Medicaid waiver services.
- Developmental Disability means a disability that is manifested before the person reaches 22 years of age, often attributed to neurological conditions when such conditions result in significantly impaired intellectual functioning and/or adaptive functioning.
PASAs may provide approved services through the Home and Community-Based Services for people with intellectual and developmental disabilities (HCBS-DD,) Home and Community Based Services Supported Living Services (HCBS-SLS), and Home and Community Based Services, Children’s Extensive Services (HCBS-CES).

Does FTA offer virtual services?
FTA offers both in person and virtual services to initiate services and any on-going communication with clients/families and care providers. However, FTA care providers are not able to provide any of the services offered virtually (ie: respite, mentorship, et al).
