Our Team

Family Together Alliance is able to provide services under the CES/SLS Waivers through HCBS Services

• Respite Care is for families with a primary caretaker whom may need a break from the stress or pressure of taking care of a special needs child/adult. 
• Respite care can be provided in a variety of settings, including the client's home and/or community settings, such as a park. 
• Respite care cannot be provided in lieu of daycare when a caretaker/parent is working for a child 11 yrs or under.
• Respite care can be used for a child 12 yrs or older when a parent/caretaker is working.
• Respite care providers need to be 18 yrs and older. 

• Community Connector services assist the special needs child while outside the home, where he/she is able to interact with others in environments that are different or may not be considered as "safe and comfortable" as what they are used to inside their home. 
• Services can be provided at locations in the community setting, such as retail stores and restaurants. Care providers support the client as they build natural supports and relationships within their community. 
• Community Connector care providers need to be 18 yrs and older.

• Homemaker Services consists of typical household tasks for the child/adult receiving services by performing them (Basic Homemaker Services) and teaching the child/adult to accomplish the tasks within their ability and encourage a timely completion (Enhanced Homemaker Services).
• Tasks include but are not limited to dusting, making the bed, picking up the client’s room, vacuuming, folding clothes, putting away clothes and help with doing the laundry. 
• Enhanced Homemaker services include the same as Basic Homemaker but in addition provides more thorough cleaning training, by encouraging the child/adult to learn and complete a more thorough cleaning according to their abilities. 
• Homemaker care providers need to be 18 yrs and older.

• Mentorship care providers will provide adult clients the ability to self-advocate through the use of instruction, providing real life experiences, role modeling and verbal prompting. 
• Mentorship care providers will include assistance/role modeling in such activities as: interviewing potential providers, assisting/role modeling with proper phone etiquette, self-advocacy, helping complete job/school applications, engaging in social activities (ie: sports team, clubs, etc) and understanding health and safety issues to help with navigating life experiences. 
• Mentorship services will be provided in community and home settings- based upon need at the time (IE: assisting will phone calls will be completed in home setting and engaging in social activities will be completed in community setting). 
• Mentorship care providers need to be 18 yrs and older.

• Personal Care care providers will assist adult clients with activities of daily living.
• Personal Care care providers will include assistance/role modeling in such activities as: showering, brushing teeth, overall grooming, preparing meals, exercise, etc.  
• Personal Care services will be provided in the home setting. 
• Personal Care care providers need to be 18 yrs and older.
